Social Work at CHI St. Alexius Health is a critical component of primary care that contributes to better care for your loved one. Preparation for each patient’s hospital discharge starts on day one of their time with us.
The patient’s care team meets daily to review their progress towards discharge.
They will add information to the patient’s discharge folder throughout their stay, including written discharge instructions, so each patient understands their responsibilities of managing their health when returning home.
